When families come to Nexus, they’re rarely looking for a single service. They’re looking for reassurance that whatever happens next, there will be someone who understands their situation and can support them through it.

That’s the thinking behind our decision to bring nurse-led complex care, or TDDI (Treatment of Disease, Disorder or Injury) care, into our offering.

For us, this may be one of the most important developments in our history as a business.

For a long time, home care in this country has been delivered in silos. A family might start with a few hours of home care support, only to find that as needs increase, they’re pushed towards a completely different provider for anything more clinical.

That handover point is often where trust breaks down. New faces arrive, new processes begin and the relationship a family has built over months or years starts again from nothing.

We didn’t want that to be the Nexus Care experience.

A pathway, not a series of separate services

Our existing home care and live-in care services already support people from the earliest signs of needing extra help through to round-the-clock care in their own home.

What’s been missing is the ability to go further, to support individuals with genuinely complex clinical needs such as tracheostomy management, ventilation, complex wound care and long-term neurological conditions, without asking them to leave Nexus to get it.

Bringing registered nurses and a clinical lead into the business means we can now offer that support directly, working alongside GPs, consultants and therapists as part of a proper multidisciplinary team.

It also means we can support end-of-life care with the same continuity, so that when a family reaches the most difficult point in their journey, they’re doing so with people who already know them.

That continuity of care is the whole point, as it means that care coordination, family liaison and clinical oversight don’t start again at each stage, adding new delays or complexity.

They carry through, because it’s the same organisation, and often many of the same familiar faces, supporting a person from the moment they first need help to whatever comes after.

That means we know that the little things that matter most to your loved ones can continue to make them feel comfortable, regardless of how their health and mental capacity change.

Why this matters now

Demand for complex, nurse-led care at home continues to grow, driven by hospitals discharging patients earlier and by more families actively choosing to keep loved ones at home rather than in a clinical setting wherever possible.

Families navigating CHC-funded packages and complex conditions need a provider who can step in at short notice and sustain that support for as long as it’s needed.

We’ve spent the past months building the clinical governance, staffing and partnerships needed to do this properly.

That work is now nearly complete and we’re proud to confirm that nurse-led complex care will fully launch in early September.

However, we are already working with several care commissioners to get new care arrangements in place for people with a wide range of complex care requirements.

What families can expect

From September, the full Nexus pathway will run from early-stage home care through to live-in support, nurse-led complex care and end-of-life care, with the same care coordination and family liaison running throughout.

Individuals won’t always move through every stage in order and needs don’t always move in one direction, but wherever someone enters that pathway, they’ll be entering an organisation built to support them through life whatever comes next.

This has been a significant investment for Nexus Care Services, but it reflects something we believe strongly – that consistency of care isn’t just a nice-to-have.

For families dealing with some of the hardest moments of their lives, it can make all the difference.
