In the early stages, many people retain a great deal of independence, and the desire to remain at home in familiar surroundings is completely understandable.

In fact, being at home, with its familiarity and routines, can really help to eliminate some of the confusion and uncertainty that people in the early stages of dementia feel.

While the traditional view of some might be a more immediate move to residential care, with the right support in place, it is entirely possible to stay at home until circumstances change further.

We know that dementia presents differently in every person, which is why our team approaches each client as an individual, taking time to learn their history, preferences and the routines that bring them comfort.

Support that starts early and stays flexible

One of the most important things families tell us is that they wish they had sought support sooner.

Early-stage dementia does not always require intensive input, but having a familiar, trusted carer in place from the outset makes a significant difference to how a person adjusts over time.

It can also provide reassurance and respite to spouses or other family members who would otherwise be responsible for all of their care.

Our care packages are designed to be flexible from the start and while an initial package might involve a few hours of companionship and household support each week, as needs develop, that package can be scaled up.

This may mean adding personal care, medication support, overnight cover or live-in care.

Beyond that, our soon-to-be launched nurse-led care can provide a higher level of clinical assistance as a person’s condition deteriorates.

This ensures that you continue to receive consistent care and certainty, without the disruption of changing providers or starting relationships from scratch.

Ongoing assessment, not a one-off plan

A care plan written at the point of initial assessment will not remain the right plan indefinitely.

Dementia is a progressive condition and the support someone needs at six months may look very different from what they need at two years.

At Nexus, we carry out regular care assessments to ensure that our provision remains appropriate and responsive.

Our team monitors changes in a client’s condition, communicates openly with family members using the latest technology and adjusts care plans accordingly.

This means families are never left in the position of having to identify a problem themselves and request a change, as we are already looking ahead.

Our recent CQC registration for TDDI (Treatment of Disease, Disorder or Injury) will allow us to extend this further, enabling us to create a single care pathway that evolves with the client rather than requiring them to move between services as their needs increase.

Remaining at home for longer

For many people with dementia, remaining at home for as long as possible is a deeply held priority.

Familiar environments, established routines and the presence of loved ones all contribute to wellbeing in ways that are difficult to replicate elsewhere.

With the right care package in place, that wish is far more achievable than families often realise at the point of diagnosis.
