Community care will provide you with support in your home and in your local community while helping you to maintain your independence and freedom of choice. We provide support on a regular or ‘as and when’ needed basis.
What does community care include?
Community care is for you if you need help with preparing meals, shopping, medication support, light domestic duties or attending appointments.
If you feel you need someone alongside you to achieve these tasks, we are here to help.
We can help you build your confidence and stay connected with the community by providing company and assurance while completing everyday tasks.
You may feel that what used to seem like a small task now makes you feel overwhelmed. We are here to help you reduce feelings of stress and worry about any tasks you may need to complete. We can also help if you have mobility issues with the physical element of getting around.
You may feel that you don’t need support due to already being independent and not needing full-time care. However, community care will help you to maintain this independence and can provide you with companionship while going about your daily life.
Your carer will be there for you as and when you need them on an as required basis.
Our community Care Division has access to a wide range of partner businesses which means we are well equipped to help you.
This includes partnerships for building work, general maintenance, wet room conversations and the installation of stair lifts. This allows us to help you with personal safety as part of your care plan.
How will my carer be chosen?
We will carry out an initial, free, no obligation assessment in which we will gain an understanding of your personal needs, preferences, and requirements. We will then be able to match one of our carers to you who will be able to support you with this.
This will ensure your carer is someone who understands you and your needs. We value building relationships between you and your carer to ensure you feel comfortable and relaxed.
Once you have a tailored, personalised and person centred care plan in place, you will feel more comfortable that you have the support in place that you need, whenever you need it.
