How to introduce extra support to your loved one’s life

The level of support a person needs differs greatly between people, so there is no one size fits all approach.
At Nexus Care, we firmly believe that for care to meet someone’s needs, empower them, and enhance their quality of life, the person should be at the centre of every decision.

The need for some extra support is not something you can plan for.
You, or your loved one, may have had an emergency trip to the hospital and need some extra help adjusting to life at home.
Especially with the Homecare Association recently finding that 63 per cent of people think patients are being discharged too soon, getting that extra help can be the difference between recovery and readmission.
Or your loved one might need help with their daily tasks and getting out and about as they get older.
Regardless of the reason, you need to know that you can get the support you need quickly, which is why we prioritise assessing people as swiftly as possible.
As a standard, we promise to conduct a free assessment within 48 hours of the initial enquiry so that you can get the help you need.
Following an in-depth assessment with our team, we craft a personalised care plan to suit an individual’s needs.
Care plans are developed in collaboration with the individual and their family, taking into account their unique preferences, routines, and history.
This involves tailoring the best time for calls and when the person needs support, as well as considering the person’s physical, emotional, social and psychological well-being.
For example, your loved one may require our Home Care support in the mornings to help get ready for the day but would also like help to get out and about, through our Community Care or joining Nexus Events.
It’s also important to know that this plan is not set in stone.
Naturally, the needs of the individual will change over time and our care plans are designed to adapt to this.
We conduct regular reviews with our clients and their family at least every 6 months or when changes are needed.
From our experience, we know how important it is to get help quickly and that’s why we try to accommodate the needs of the person as soon as possible.
Our responsive team focuses on getting the right care in place, usually within 7 days of the assessment, or at the client’s request.
As one of our clients, Piers, said: “When I ended up in hospital for a month I knew when I was discharged, that I would need additional help to get me back on my feet. I phoned John on the Friday afternoon that I was coming out and that same evening he had carers in place to look after me – that’s how responsive they were.”
